Once we have received your imaging, our patient advocate will contact you to set up a complimentary phone call with one of our surgeons to determine your preliminary candidacy for Minimally Invasive Spine Surgery.
Please upload a zip (compressed) file of your MRI or CT scan images.

To create a zip file (PC, Windows 7 and below):
1. Load your CD into your computer.
2. While holding down the Windows key (start), press the letter E to Open ‘My Computer’.
3. Locate your CD file. Right click on the file and select ‘copy.’
4. Click on your desktop, then Right click and select ‘paste’.
5. Right click on the newly created file on your desktop, and select ‘Send to Compressed (zipped) folder.’ A new file ending in .zip will be created.
6. Once your zip folder is created, upload it here.

To create a zip file (PC, Windows 8):
1. Load your CD into your computer.
2. Open File Explorer by swiping in from the right edge of the screen, tapping Search (or if you’re using a mouse, pointing to the upper-right corner of the screen, moving the mouse pointer down, and then clicking Search), entering File Explorer in the search box, and then tapping or clicking File Explorer.
3. Select the file or folder, tap or click Share, and then tap or click Zip. A new compressed folder gets created in the same location with the same name as the file. To add new files or folders to your compressed folder, you can drag them to it.
4. Once your zip folder is created, upload it here.

To create a zip file (MAC):
1. Load your CD into your computer
2. Right-click (or Ctrl-click) files you want to compress inside Finder and choose Compress Filename. The newly compressed files carry the .zip extension. The archive is created in the same location as the original file and is named originalfilename.zip. (You can also choose File→Compress. If you compress a lot of files at once, the archive takes the name Archive.zip.)
3. Once your zip folder is created, upload it here.
